Admission Precedures
Upon arrival at The Doctors’ Hospital, Inc., patients must undergo medical triaging at the Emergency Room for assessment.
Patients or relatives will proceed to the Admitting Department to complete the admission process. The Admitting Staff will ask the patient to present the following:
- Doctor’s admission order sheet - This will indicate specific requirements for the patient’s case. In the absence of the doctor’s admission order sheet or a doctor known to them, a walk-in patient may be admitted through the Emergency Room.
- Letter of Authorization (LOA) - This is for Company or HMO/PPO Sponsored Accounts. Patients without LOA will be automatically considered as private-paying patients and an initial deposit will be required. Those who submit the LOA during their confinement may be re-classified from private to Corporate or HMO/PPO sponsored accounts.
- Privilege Card - From patients who have previously been admitted to the hospital.
The Admitting Staff will gather all the necessary information for the admission. The patient or relative will then be advised to fill out the necessary forms and provide one (1) valid ID for data verification.
For HMO admissions, the Admitting Staff will coordinate with the respective Health Maintenance Organization (HMO) to verify the patient’s eligibility and coverage for hospitalization.

The relative of the patient will be given a form to fill out, and will be advised to proceed to the Claims Section at the Billing Department for other necessary requirements to be completed within the day. Below is the steps in accomplishing PhilHealth requirements:
- Duly accomplished Original CF1 (Member/Patient – Part I, II, III & Employer – Part IV)
- Duly accomplished Original CF2 (signed by attending physicians Part II No. 10 – & Patient/Representative – Part III – B)
- Generated Philhealth Benefit Eligibility Form (PBEF)
- If the PBEF indicated YES – no need of nos. 1, 4 & 5 only CF2
- If the PBEF indicated NO – accomplished the needed requirements indicated in the PBEF.
- Certificate of Contributions or Receipt of Premium Payment
- At least 3 months from confinement backward (3/6 rule)
- Properly Accomplished Philhealth Membership Registration Form (PMRF) for the undeclared dependents
The patient will be asked to sign consent forms for hospital care, exclusive supply of medicines, limitation on outside diagnostic reports, release of information to insurance companies and/or patient's employer, and waiver of responsibility on loss of valuables. The Admitting Staff will then inform the necessary departments of the new patient admission.
The Admitting Staff will present the available rooms, letting the patient or relative decide on the room of choice. Once the patient or relative has decided on a room, the Admitting Staff will coordinate with the Nursing Staff to ensure that the assigned room is ready and available prior to the patient’s transfer.
Once all the necessary information has been completed and encoded into the system, the admitting papers along with the ID wristband will be forwarded to the ER Nurse. The patient will then be prepared for transfer.
